Have the staff of death but do not now where to go. Also found a lock behind the lich's main room (library) but my keys didn't work. Is this the way to Madda?

Does the lock look like a carved head? I'm not exactly sure, but I do know that Mabba Madigs room is at the top of a flight of stairs. You might have passed it before u came to the Lichs room. You should hear her moans anyway,

Madda Mabbig is found way at the bottom of Skull Castle in the west portion from the bridge/lever puzzle. She's behind a locked door which stone faces on the side. Use the staff on the stone face to open the door.

You can actually defeat Mabbig before you open the door. Usually some little bit of her is sticking through and if you can target that, you can damage her and kill her without her ever having a chance to harm you.
