Where do u put the medallion of the lost followers to get the weapon?

You have to take the medallion back to the Kuldahar graveyard and click on one of the headstones in the outer part of the graveyard.

However, once you click on the headstone, it will initate the battle with the Lost Followers so be very, VERY ready for an incredibly nasty battle.
