Can't help you with the custom monsters, but I can help with the custom portraits.

Okay, here are the general rules:

Custom portraits have a large and small portrait, both of them must be in 24 bit BMP format and adhere to the following file naming convention.

xxxxxxxL.bmp = large portrait 110 x 170 pixels in size
xxxxxxxS.bmp = small portrait 38 x 60 pixels in size

the xxxx's can be anything you want but can not be more than 7 characters long.

Typically, what you you need to do to make reasonably good custom portraits is to find a high quality scan and resize and crop it to the above dimensions. A good Photo editor such as Adobe photoshop or JASC photoshop pro is essential.

Once you have the large and small photos created, you just need to create a new folder within the main Icewind dale folder called "portraits". Drop your photos in there. When the portraits pop up during character creation simply select "custom" and that will send you to a dialogue box where those new portrait files are located. You can also change the portrait of an existing character by selecting "customize" from the character sheet.

hope this helps.
