The procedure is identical - just create a folder called Portraits in the main directory for IWD/HOW and copy your portraits into that folder. You just need to make sure that the large and small portraits are tagged as xxxxxL.bmp and xxxxxS.bmp respectively.

Couple of things to be careful of: the IWD and BG1 portraits are not the same size. I don't remember the exact dimensions but they are close to each other but not exactly the same. I've been able to use my custom portraits in both games without trouble but that may have been shear luck. ALso, make sure that the file name of the portrait is less than 7 characters excluding the size designator.

Other than that, there are no special tricks.
