Help I'am stuck. I have killed everything in the pass, even in the cave and I can't find my way to Kuldahar. Is there something that I am missing or did not do.

Look to the far right of the map. You should see a mill. Cross the bridge, then go as far east as you can. You should see the "exit" icon that will take you to Kuldahar.
