Soulreaver +4 cannot be upgraded AFAIK ...

Yes daan is correct soulreaver is a +4 weapon. If you saw someone with a +6 one it must be from a mod.In case you don't know the soulreavers location it is...

go to the kuo-toa lair in the underdark and make a animal sacrifice(spider etc) on the statue of demogorgon. Some demon nights will be summoned and on one of there bodies is the soul reaver, there's also a very nice armour as well.

The Soul Reaver +6 is in the Item Upgrade Mod.
In order to fo the upgrade, you need the Soul Reaver +4, the Heart of the Damned, the Carsomyr, and 20,000 Gold.
