What do the map colours mean? Some are faded, some are solid, some are solid but outlined in blue.

Faded means an area that's known to you but you can't reach it yet. You need to be in an area next to it to be able to travel to it.
Solid is an area where you've already been, you can travel to these at any time, no matter where you are.
And a blueish area is one you've just 'discovered' by walking out of the adjacent area, but you have't been to it yet. You can also reach these from anywhere.

Both the faded and the blueish areas change to solid once you've been there.
