I've created several custom items for my BG2 game, most of them specific to certain characters, but some of more general use. Herewith, one example:

The Amulet of Bugs

This amulet acts in all respects as a normal Amulet of Protection +1 (except that it's protection ability stacks with others), but it has one additional power. By stroking the black cockroach hanging from the chain and uttering the mystic incantation "yech, bugs!" the character may instantly cast the Insect Swarm spell. This ability is useable once per day, and the recharge is enabled by stroking the cockroach a second time, and declaiming "We don't need your stinking Druid!"

It's kind of useful, especially for those who really can't stand Jaheira.
