I killed the main beholder w/ the rift defice. Afterwards, the device disappears from the inventory! I can't get a reward from the Avatar for this quest because he won't talk to me. What did I do wrong?

IIRC, the rift device disappears from your QuickSlot and appears in your backpack... all I can think is maybe your inventory was full and the device was dropped. Did you search the floor?

If it's not in the floor, the codes are:
MISC5A - Rift Device Part 1
MISC5B - Rift Device Part 2
MISC5C - Rift Device
