I was fighting Perth the Adept and he cast Project Image I think (the spell which created a lower level version of your self), however he cast it at the exact same time as I kiled him with a critical hit. This created instead of a duplicate of him, a flying chunck of meat which then started attacking me with spells, I think its the most odd bug I have ever seen. Quite funny though. It was weird though, its voice set for casting spells was female.

wow, lol. I've come across similar things, when timing has managed to create weird circumstances (although the only one I can think of now is Delon following me to the slums district, not that spectacular)

Delon following you to the slums district isn't actually a bug. Normally he is found in the government district but after a set amount of time if you have Minsc in your party he will seek your party out.
