Hammer of thunder bolts - in the athlanka seweres (requiries key from tazok)
frost giant belt - Kuo tuo prince in the underdark (kuo tou lair)
gloves of strength (18/00) - Planar sphere

Gauntlets of ogre power: on halfling in planar sphere
scroll: shadow dragon, temple ruins
girdle of FGS: Kua-Toa Lair, eastern underdark
hammer of thunderbolts: mind flayer lair, sewer, temple district

Just a minor elaboration, the Gridle of Frost Giant Strength is obtained from a Demon Knight in the Kuo-Tuo Lair, not from the Kuo-Tuo Prince.
