I have a blue gem that seems like it used to be in a sword. I think it belongs to the Long Sword: the Equalizer but I'm not sure. Any help would be greatly appreciated.

It's called the 'Pommel Jewel of the Equalizer'. That was a bit of a giveaway from my perspective. You can't reforge it until you get the other two pieces, which are in the Underdark.
