Scroll - shadow dragon in temple ruins
Hammer - mindflayer dungeon under temple district in the sewers, you will need Tazok's sewer key to access this, he is in windspear hill dungeon
Gaunlet - Enfu (some evil halfing) has it in the planar sphere in slums
Girdle - Kuo-Toa dungeon in Underdark, you need to summon animals under the Gorgon Statue to spawn demon knights, one of them has the belt
