You can only do this a few times before the characters start getting a default number of hit points.
Fighter-class player character: use innate holy might before you level up to get one more hp.
Clerics: use innate holy might to get 2 more hp.
Other characters: use potions of fortitude (unless they are like Yoshimo, Amoen, 16 con in a non-warrior class).
