Anephestas shrine, its locked, i must be stupid but how do i get inside??

I you mean the front door to building - you need to chat to the lich in skull castle first, he's the one who sealed the place, only he knows how to get in.

what does he say, when he explain how to open the seal, maybe i have missed something.

Okay, big spoiler here....

The lich's staff is what he used to seal the door. "Use" the staff to unseal it.

The staff is also used in Skull Castle to "unseal" another special door.

Remember that your journal records all conversations, if you forgot what the lich said you can just look it up.

If you didn't talk to the lich enough to get all the information then you would have a problem, but that's what the board is for.
