where do you find the rest of the wave shaft and the hilt and pommel stone for the blade of the equalizer?

The wave blade is on the Prince in the Sahuguin city. The pommell jewel is in Irenicus' dungeon. The hilt is carried by one of the Githyanki that track you down. (I believe. I'm not sure about the hilt. The others I'm sure of.)

Actually the hilt from the Githyanki is the hilt of the silver sword.
The hilt for the equalizer is on a Beholder in the Underdark.
