Just one more suggestion, it might be just be that info on the Sahuagin city is corrupt on Disk 4, If you take the portal in the assylum instead of Saemon's ship you will miss the city altogether. All though doing this will lose you some great equipment the best of these items being the gauntlets of crushing (unless you are a monk these dont matter) and the cloak of mirroring (in my oppinion the best cloak in the game). But then again it could be your disk 4 and this wont work either, but it's worth a try

Sounds like the problem many of us had with somewhat older systems at the time. It turned out that either our cd-roms were going out and were getting to the point where they were just unable to read disks properly at times, of course it just got worse and worse until it wouldn't read any disks at all. I went through the same thing you describe, asked for cd 5, then 6, very disappointing. Solution, needed new cd-rom drives. Some other people just had the problem where the reader head just needed cleaning. So you might want to try that as a first case solution.
