I've heard there is a known bug that if you release an NPC when they have a level-up available and you don't level them prior to releasing them that they disappear forever. Could this be what is behind some of the odd disappearances reported in this thread? I realize it's probably not the reason for all of them but perhaps some?

I can say for certain that neither Myles nor Madras were at level-up when I left them (that is to say, they left me, at the Swamp and the Rift, respectively).
